If you are looking to retrieve your records or have records transferred to Nashua Eye, please print form, fill out and send it to the medical records department.
Surgery Forms
If you are scheduled for surgery, please review the below forms.
1) Please review the surgery instructions
2) Fill out the Anesthesia Evaluation and return it to Nashua Eye, either mailing it back or dropping it off.
3) Please bring the Primacy Care Provider (PCP) Clearance form to your primary care doctor for clearance before surgery.
