Referrals
To refer a patient, please either enter patient demographics below OR fax patient demographics to to 603-882-0556. Please include reason for the referral and identify if a specific doctor is requested. Our faxes are checked daily and patients can expect to receive a phone call within 3 business days of receiving the fax. If the referral is more urgent, please call our main office at 603-882-9800.
